The ideal quote request form for ads has four to six fields: enough for sales to reply with confidence, and few enough that visitors do not give up. The exact number depends on what you sell and how much filtering you need. Below are straight answers to the most common questions.

How many fields should the form have?

Start with name, work email, phone and a text box describing the need. Each extra field tends to cut submissions, so add only what changes the sales reply. Ask yourself: if this information is missing, can I still answer the first contact? If so, leave it for the conversation.

Which fields should be required?

Required: name, email or phone (at least one channel) and the description of the need. Optional: company, job title, quantity, deadline and attachment. Making company required helps filter curious visitors and consumers, which matters for operations with a minimum order.

Should I allow file uploads?

For made-to-order services such as machining, cutting, CAD design and 3D printing, yes. A drawing, a photo of the part or a spreadsheet speeds up the quote and signals a serious buyer. Keep the field optional, accept common formats (PDF, DWG, STEP, images) and state the maximum size. Say that files are handled confidentially.

More fields or more volume: how to decide?

It is the old trade-off between quality and quantity. If your sales team is stretched and each quote costs hours, a longer form filters better. If the team replies fast and deal size is high, accept a short form and qualify by phone.

  • Short form (3 to 4 fields): more submissions, more noise, good for high-ticket deals and a fast team.
  • Medium form (5 to 7 fields): a balance, the most common in industry.
  • Long or multi-step form: fewer submissions, higher quality, suited to complex quotes.

Do multi-step forms work?

They can work well. Step one asks for contact details only; step two covers the request. Anyone who completes step one has already left contact data, even if they quit later. Test against a single-screen version to see which yields more qualified quotes.
